Request for conciliation form
The Request for Conciliation Form can be downloaded in pdf here. If you require a hardcopy Request for Conciliation Form, please contact the ACCS.
Completed forms must be sent to the ACCS. Please read the form carefully and ensure that:
- All details provided are correct,
- You retain a copy of the form for your own records; and
- The form is signed by the person requesting conciliation.
If you require assistance in completing the Request for Conciliation Form, please contact the ACCS.
Video/DVD and booklet
A Conciliation Service video/DVD and booklet is sent to everyone attending a conference for the first time. They aim to explain the process of conciliation and to help parties coming to a conference to be prepared.
